From Castlevania 64, a cut scene where, after just defeating a vampire boss, the girl he was just feeding on wakes to finish the job.

In Castlevania for the NES when you defeat Dracula and big surprise! he's not dead yet, then that damn "bat" monster appears.

In Majora's Mask for the N64 when you take a long time with the game and you look up in the sky and the damn moon is almost on top of your head.

Vampire: Bloodlines had so many...
Pretty much the entire run through the haunted hotel.
The 'Crack House' level where you get covered in a thick blanket of zombies if you're not careful.
Same level, finding out you're up against the only vampire who can snipe with a shotgun.
Grout's mansion, realising the peons that are on fire now cause HEAVY damage.
Not being prepared for any of the 'tests' in the Fu Syndicate building.
And finally... Getting all the way to the end and accidentally going down the conversation path to the Prince LaCroix ending. Anyone who's got that ending will know why it's an 'Oh, SHIT...' moment...

Unless you wait til the fight starts, then unplug your control pad from port 1 and plug into port 2. Heh. You can control your character without Psycho Mantis being able to mess with your visuals or anticipate your actions.

Kojima's great at throwing in those sort of moments.
In MGS2 when the Colonel starts with the weird Codec calls, that initial reaction got a real Oh....shit from me, especially as it was about 1 in the morning when I was playing it.
Then seeing all the RAYs come towards you when you're on the roof of Arsenal.
Also, on COD4 when the ship starts exploding in the first mission. Especially as I was playing in on a mate's 24-inch HD tv. Then the water started pouring in and you had a mad rush back to the helicopter, which you had to dive onto.
Then the nuclear strike in a later mission

rainbow 6 vegas 2.

you rely totally on your team for the whole game and in one of the last levels you have to finish it on your own.............IMPOSSIBLE!!

Condemned: CO in that damn houses basement. Every time you put away your weapon to try to find that piece of writing another enemy appears. Its honestly like theres no end to them. Sure a single tazer shot drops the little bastiches but theres no end to them and theyre all creepy and crawling. ><

Got to be when u attack a random chicken in zelda and they descend in their thousands trying to claw your face off. I was running around saying "MAKE IT STOP, MAKE IT STOP !!!"
