Having gotten that out of my system, LSD would be a close second. It also makes you hyper for about 6 hours, which is even more than caffeine will get you. It's just, you know, the bits where you drown in an endless, cold, black ocean kind of offset the practical benefits.
Ah, LSD. I'd almost forgotten about that. I hear it's actually one of the safest drugs out there if it's good.
I guess I should specify that the supply of your drug of choice would be absolutely clean and prime quality.
I don't want concerns such as quality to get in the way of making a proper judgement.
I haven't tried LSD though, so it's hard to commit to a lifetime with the stuff without knowing its effects intimately.
I guess a lot of it comes down to whether the occurence of "bad trips" is due to poor quality in the drug or if it's just an inherent thing that you can't get around.
No, drowning in an endless, cold, black ocean was not a bad trip. It was what happened when I drank cola the first time I took acid. It only lasted for about 3 seconds and was more interesting than bad. I'm just saying that LSD works as a drug to keep you awake and active, but that the occasional bit of confusion might impede your daily functioning more than caffeines negative effects. That's why I made LSD my second choice.
Ah, yes. I assume trying to replace caffeine's practical effects in your daily life with LSD would end pretty... weirdly?

If I were to chose LSD, I'd probably keep the use confined to the weekends.
And yes, LSD is quite safe. Neither physically nor mentally addictive and not medically harmful. It's safer than any legal drug (including weed, if that fits your countrys definition of legal). For the life of me, I can't understand why it's illegal at all. I assume paranoia and the all-encompassing fear of old voters led to it being outlawed, but I'm not sure why that still holds up.

Heroin. I get it on prescription so I suppose it comes under the "available for medical purposes" clause anyway, but hell, I still choose that.
Edit: Secondly, if I was making the decision for the whole of humanity I suppose I'd go with something different since I still get what I want under the "medical purposes" bit. So then I go for...umm...probably caffeine, I like tea and I can't think of any other drug I'd want to do.
This was probably a bad idea for a thread, with most drugs being either medical or illegal.
I myself would have to go with caffein, like everyone else here apparently, since it's already the only drug I intake. I didn't realize how addicted I was to the stuff until I went a day with only drinking water and started going through withdrawl. I felt like a dizzy tranquilized slug with a pulsing headache.
I have no interest in caffeine, alcohol or nicotine whatsoever. Legal drugs has never really been my thing.
Fact is, drug usage doesn't appeal to me at all. I have the same problem with them as I have with religion, namely that they're distorters of reality. If I get sick of reality, I'll just escape it rather than distorting it, by e.g. drawing, gaming or listening to music. That way, I won't end up with a knife with blood on its pointy end, figuratively speaking.
So yeah, I'll stick with my ADD medicine.
Again, caffeine/stimulants in general.
I don't even like coffee. I wouldn't even mind if sodas were drained from caffeine.
But simply it would kill the energy drink market. I am still convinced that if you get hooked on then they will slowly poison you, but I can't imagine a modern world without stimulants like caffeine, taurine and whatever.

I don't really care about caffeine. I have occasionally smoked weed, but it really was occasionally, and even then not in over a year.

I wouldn't have any drugs. But I guess if I had to choose, I'd try introducing supplementary doses of Serotonin or Dopamine into my body.

I love my coffee but I guess I could survive without, alcohol is ok for socializing even if I don't actually like it as a drunk, weed was fun for awhile until you realize how boring it makes you...
I guess I'll say MDMA. I tried it for the first time about a week ago and I've never felt a high quite like it. It's powdered love.

Caffeine. I'm properly addicted- I tried to go without coffee for a few days once and ended up getting the shakes :S Seriously, I drink way too much of the stuff. We're talking starting the day with a whole pot of coffee -about 3 mugs- or pot of tea -about 5- (damn, not helping the British stereotype :P ) and spiralling from there.
Not that I'll stop any time soon though. Sweet, delicious caffeine...

In cases of much larger overdoses, mania, depression, lapses in judgment, disorientation, disinhibition, delusions, hallucinations, or psychosis may occur, and rhabdomyolysis (breakdown of skeletal muscle tissue) can be provoked.
It sounds like ingesting those quantities has quite the uncomfortable side effects.
I've consumed half the lethal dose at once and all it did was make me feel extremely ill.

Well I'd have to jump on the caffeine bandwagon seeing as medication and illegal drugs aren't allowed.
If Painkillers are some of the drugs I can't have, then one of those is my choice, probably IB Profin. Otherwise I would pick alcohol, so long as I can have caffeine free versions of mtn dew and monster that taste the same. If I can't, then it's caffeine
A generic almost universally effective painkiller that has other health benefits is tough to beat.
I for one can live without caffeine, and most people who think they can't need to step down off of it for their own good anyways.
So drink a cup of willow tea a day.
I could easily give up caffeine, it's up in the air between weed and alcohol, with alcohol already being legal, it almost seems like a waste to pick it. I'd probably go with alcohol still. That being said variety is the spice of life and removing every other drug would make the world very boring, even for you straight edge kids because without other people being weird and stupid, you'd have no one to laugh at and for some of you, to look down upon.
Caffeine. Can't live without it, and since pain medications would be available at need, that would be my choice.

Caffeine is the hands down winner. All the other stuff is great (to varying degrees and some notable lows), but without caffeine you're giving up tea, coffee and any number of soft drinks, y'know, those things you have daily between weekend excursions on the big stuff?
I'd probably just stick with my current medication.
I'll go with caffeine. I can live without an occasional glass of red wine or a cigarette, but things like coffee and Red Bull make my life that much more worth living.
A rather odd question, really. What with the medical issues being taken care of (I was gonna go with penicillin) and the resolution of other issues, such as the definition of a drug ("a substance that has a physiological effect on the human body") being broad enough to cover, well... everything, really, from food to water to breathable air (and if you don't think those have physiological effects, go without them for slightly less than the lethal time period and add them back for some startling results)... well, the answer seems to be determined almost entirely by the holes in the question.
Caffeine, for example, seems to be the most popular answer... but how many people have said that because they actually, to an extent, need it for medical purposes? If your boss wakes you up at three in the morning and says you have to start an eight hour guard shift right now (which happens, for those of us in the lower ranks of various militaries) is that cup of coffee you grab on your way a luxury, or is it a very important medicinal remedy for the medical condition of falling asleep on the job? I mean, sure, you could try to fight the encroaching sleep the old fashioned way, but you could also try to get over many severe infections on your own, with similar chances for success and similar levels of stupidity. Where do you draw that line?
Just strikes me as odd, I suppose. Seeing as I don't smoke, don't drink, and don't use any sort of illegal substance (let alone more than one, which would make this an actual choice) I suppose this question just isn't for me.
